In an amazing story of passport identity theft, David Garrett, a New Zealand politician and a barrister, before entering parliament in 2008, admitted to first applying for a birth certificate in a dead child’s name and then using that birth certificate to obtain a passport in that child’s name.

Garrett claims to have read the 1971 book ‘Day of the Jackal’ in which a character in the book does just that. Garrett says it was a “harmless prank” and says he was “curious to see if such a thing could be done”. He now says that he is sorry for his “thoughtless actions” that have caused pain to the family of the deceased boy. Garrett says the passport expired many years ago and that he “never used the passport for any purpose”. This passport fraud was found out because the New Zealand authorities were looking into passport identity theft used by two Israelis thought to be Mossad agents.

The Phoenix Arizona Police Department put out a good 13 page PDF that gives you information to assist you with correction of identity theft. This Identity Theft Victim Report is tailored to people in Phoenix, Arizona but the information is useful to anyone who is a victim of Identity Theft.

The report has 5 steps.

1. Contact your bank and other credit card issuers.
2. Contact all three major credit reporting bureaus.
3. File a report with the Federal Trade Commission.
4. Contact creditors involved in the Identity Theft by phone and in writing.
5. Submit the Identity Theft Affidavit and copies of all information and records obtained from the creditors with regard to the fraudulent accounts to the police department.

At the end of the report they list “Documents for Prosecution” which will help a police department begin an Identity Theft Investigation.

1. What documents you need if an identity theft has occurred with an Existing Account.
2. What documents you need if an identity theft has occurred with a New Account fraudulently opened in you name.
3. What documents you need if an identity theft involved someone using your personal information For Employment.
4. There is a ID Theft Affidavit which helps you lay out to the police what happened.
5. There is a Fraudulent Account Statement, which allows you to list accounts that are fraudulent.
6. They have two  Sample Dispute Letters you can use to send to a creditor when someone committed identity theft against you.

Overall this is a helpful document in working your way through your identity theft situation.
